The Providers and Staff at Lifestages - Samaritan Centers For Women are committed to meeting with you at your scheduled appointment time. Unfortunately, sometimes emergencies arise that may keep us from meeting with you on time. We will notify you as soon as possible of any delays, so that you may adjust your schedule.

In order for us to remain on time, we also need your help. We need you to arrive ten (10) minutes before your scheduled appointment time. This will allow time for the registration process. Please notify us 24 hours in advance if you are unable to keep your appointment. We also realize that women in today's world are faced with challenges and sometimes circumstances arise that prevent you from giving 24-hour notice. If this occurs, please let us know.

Please bring your current insurance card and any required co-payments to your appointment. If this is your first appointment or it has been over a year since your last appointment it will be necessary to collect medical history information from you. You can complete this information prior to your appointment by accessing our forms page.
